Dedication plaques

Our Celebration Tree in the hospice garden, offers you a wonderful way to celebrate those who are special to you with a dedication plaque. The tree is decorated with a selection of designs for you to choose from including leaves, birds and flowers. Each plaque will be engraved with a personalised dedication in memory of your loved one.

Dedications are available for a minimum donation of:

•  £15 per month or
• £40 per quarter (saving you £20 a year) or
• £150 annually (saving you £30 a year) 

Your plaque will be displayed for one year and after that time we’ll invite you to either renew your donation to keep it on the tree, or to have it returned to you to keep. Each dedication will enable Forest Holme to continue to provide care and support to patients and families from across Poole, Wimborne, and the Isle of Purbeck.
