The Palliative Care Record is part of the Electronic Patient Record (EPR) of Poole Hospital NHS Foundation Trust. EPR is used to record people’s preferences for end of life care, holistic assessments, the continuous clinical record and the outcomes of multi-professional meeting discussions.
The Hospital Palliative Care Team has led on several initiatives to improve care. EPR now contains the following documents which are widely available – not just those for those known to the palliative care team:
- Planning ahead for end of life care – the individual’s preferences for end of life care, including place of care and other priorities
- Treatment escalation plan – to document the treatment options which may be appropriate in the event of deterioration
- PEACE – Proactive Elderly Advance Care – planning ahead for care home residents
- Personalised Care Plan for the Last Days of Life – an individualised care plan to support high quality care and communication
Primary care teams can access training in EPR via the IT department at Poole Hospital NHS Foundation Trust.
