Palliative Care In Hospital

Forest Holme Hospice has a hospital palliative care team comprising of specialist palliative care nurses and facilitators in end-of-life care who support patients within Poole Hospital.

Community Hospitals are supported by the community specialist palliative care team.

Continuity of care

Poole Hospital staff can refer to the team for advice on symptom control and emotional support for individual patients and their family whilst in hospital.

The hospital palliative care team ensures continuity of care on discharge by liaising with their community palliative care colleagues and other health care professionals in the community.

Education and support for all staff

The team also provides education and support to the staff involved in the care of people with life-limiting illness.

The team is available from Monday to Friday 8.30am to 4.30pm.
